Every day, over 1 million people worldwide are infected with STIs.
Chlamydia are bacterial infections of the mucous membranes, caused by the bacterium Chlamydia trachomatis. They primarily lead to inflammation of the urethra, genital tract, and rectum.
If symptoms occur, discharge from the urethra or vagina, as well as itching, burning and pain during urination, appear after one to three weeks.
If left untreated, chlamydia can lead to pelvic inflammatory disease, inflammation of the prostate, testicles, and epididymis, as well as infertility. In women, inflammation can cause the inner lining of the fallopian tubes to become blocked, preventing the egg from being transported.
Syphilis is a bacterial infectious disease caused by the bacterium Treponema pallidum. It typically progresses through several stages and, if left untreated, can affect various organ systems, including the skin, mucous membranes, lymph nodes, cardiovascular system, and nervous system.
Syphilis progresses through several stages and is therefore often not recognized immediately. In the first stage, a painless ulcer usually appears at the site of infection – for example, on the genitals, in the mouth, or anus – which disappears on its own. Weeks later, flu-like symptoms, a skin rash, or swollen lymph nodes may occur. Because the symptoms are mild and nonspecific, they are frequently overlooked or mistaken for other illnesses.
If left untreated, syphilis can severely damage vital organs such as the heart, blood vessels, and brain in later stages – leading to permanent damage and even death. The good news is that syphilis can be reliably and completely cured with antibiotics in its early stages.

Clinically proven effect
Clinical studies demonstrate the high efficacy of medication-based STI risk reduction. The incidence of bacterial STIs such as chlamydia and syphilis is demonstrably significantly reduced in individuals using this method. (Source: San Francisco Sexual Health Clinic; Scott H et al, CROI 2024)

Drug-based STI prevention
Condoms are a good method of protection. However, they are not the preferred choice for everyone. Certain medications can offer an effective alternative or additional protection. They can significantly reduce the risk of contracting chlamydia, syphilis, and, to a lesser extent, gonorrhea.

Post-exposure prophylaxis
If indicated, a doctor can prescribe a specific antibiotic to be taken once after sexual contact. When used within 72 hours of intercourse, it can significantly reduce the likelihood of infection with chlamydia and syphilis.

For high-risk individuals
According to clinical studies, it is effective in people (AMAB) who have condomless sex with men (AMAB). Condomless sex means that a condom is not used most of the time. This can be the case with any type of sex: oral, anal, vaginal, or front-hole sex.

Despite many years of use, no resistance to standard therapies has been observed for chlamydia and syphilis.
In contrast, gonorrhea is already highly resistant to antibiotics, which is why the antibiotic used for prophylaxis is not used for treatment. Other antibiotics are typically used in this case. Furthermore, there is currently no evidence that prophylaxis affects standard gonorrhea treatment, as it apparently does not increase resistance to other antibiotics (Chu VT et al., CROI 2024, #1154).
With other bacteria, the development of resistance is rather low with moderate use (Chu VT et al.; CROI 2024; #1154) and even with daily use is limited and only temporary (Truong R et al. 2022; doi:10.1093/jacamr/dlac009.).
In summary: Antibiotic resistance is a serious issue and should be monitored. However, when taken responsibly and under medical supervision as post-exposure prophylaxis, major side effects are unlikely. This approach has proven highly effective in significantly reducing the risk of STIs in high-risk groups who engage in condomless sex.
As with most medications, STI prevention drugs can also cause side effects or allergic reactions.
